What is CHDP Program

The Child Health and Disability Prevention (CDHP) Program aims at offering complete and periodic health assessments that can help in early detection and prevention of diseases and disabilities in children and youth from low-income families. The heath assessment by the CHDP program includes checking medical history, physical examination, developmental assessment, nutritional review, vision and dental checkup, hearing test, laboratory tests, tuberculin test, immunizations, as well as health/education anticipatory advices and guidance, and referrals for any necessary treatments or diagnosis.

This program oversees the various screening and follow up components of the federally mandated Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) program for the Medi-Cal eligible youth and children. Health assessments are offered by enrolled private physicians, community clinics, local health departments, managed care plans and some of the local school districts.

In 2003, CHDP started using the “CHDP Gateway,” which is an automated pre enrollment process aimed at the non Medi-Cal and uninsured children. The CHDP Gateway offers the entry point for the non Medi-Cal children to enroll in ongoing healthcare coverage programs via Health Family or Medi-Cal program. The CHDP Gateway is based on federal law that is found in Titles XIX and XXI of the Social Security Act, according to which, states can establish presumptive eligibility programs for youth and children.

Program Administration

The CHDP program is operated at the local level by health departments for every county and three cities. Local CHDP program undertake community activities, which include planning, evaluation, monitoring, case management, informing and offering health education materials, quality assurance, provider recruitment, and client support services like transport assistance, medical, mental and dental health appointment scheduling, encouraging the completion of ongoing health care coverage application.

It is also the responsibility of the local CHDP program to make sure that high quality services are offered to the children and youth who are eligible for the program. The CHDP program informs the target population of the program to improve their participation, and the different community agencies and residents to further improve their knowledge and acceptance of the preventive services.

CHDP program offers care coordination to help families with different medical appointment scheduling, transportation, and access to various treatment and diagnostic services.

In recent years, he’s been involved with a non profit organization known as Every Woman Counts (EWC) which provides free breast and cervical cancer screening and diagnostic services to California’s underserved populations in order to eliminate health disparities for low-income individuals.
